Murrieta Surf Risk Management Procedure

All administrators assigned to hold the position of head coach, assistant coach, board member, team managers, team assistants, or team parents must comply with Cal South rules and be Risk Management Approved before they can perform any of the above duties. 

Candidate must contact the registrar and have their information entered in the Coris database. 

STEPS to follow:

1. You will need to know your team number as this needs to be entered on the form. (Contact registrar) Complete a Cal south Team Administrator Assignment form below. 

Note: Enter the info listed below on the form:

SEASON: Enter the info listed below on the form:
CLUB: Murrieta Surf Soccer Club
TEAM ID Number: Enter your team name

Print out & sign form and email to or fax to 866-582-4111, no cover sheet needed.

If dropping off form, print out 2 copies of the form, one is dropped off as listed below, the other goes in the team book. 

2. Email the registrar, Gladis Rada, and provide the following information: 

a. Name

b. Address

c. Home/Cell phone numbers

d. Position you are applying for

e. Driver’s License number and Date of Birth

f. Picture (head shot)

Once you have provided the information above you can schedule your appointment with any UPS Store or any other place providing livescan services. Cost is approx. $20 – this is a team expense for team managers and team assistants, coaches must pay themselves. 

You will need to complete, print and sign 3 copies of a Request for Live Scan Service form (click on link below for form) to take with you to your appointment.... 

3. The completed Livescan form must be emailed or faxed to the registrar, see details above.

4. If you don't have access to email or fax, the forms these can be dropped off in the Incoming File of the MYSL drop box on

porch @ the registrar’s home: 

Drop off the following:

1 copy of the completed and signed Administrator Assignment form completed earlier (Step 1), and a copy of the completed livescan form @ 23861 Costa Mesa Way, Murrieta, CA 92562. Once your card is ready the registrar will contact you to pick up your card. 

The second Team Administrator form is placed in the manager’s book.

All Head Coaches, Assistant Coaches, Team Managers & Team assistants must complete the Heads Up to Youth Sports Concussion Training Course & upload to their Calsouth account their Concussion Certificate, which will be available at the end of the course.

HEADS UP Concussion in Youth Sports is a free, online course available to coaches, parents, and others helping to keep athletes safe from concussion. It features interviews with leading experts, dynamic graphics and interactive exercises, and compelling storytelling to help you recognize a concussion and know how to respond if you think that your athlete might have a concussion.

Once you complete the training and quiz, you’ll be able to save and upload the certificate to your Calsouth file (for instructions, see video below).

If you have already completed the course & uploaded the concussion certificate thank you. You don’t need to do anything else. Below you’ll find the links for 2 sites where you can complete the course & instructions on how to upload the concussion certificate to your account.

CDC Heads Up Concussion:

CDC Heads Up Concussion module is an online course with a duration of about 30 minutes in length. Your admins will create a profile account with their system and be sent an email confirmation to be able to start the course. Once complete, they will be able to save and print their certificate. National Federation of High Schools (NFHS):

National Federation of High Schools (NFHS) has partnered with the Center of Disease Control (CDC) to offer Concussion in Sports courses. Your admins can create a profile within that website and will quickly be able to order a course. Their certificate will always remain within that account and can be printed as well. An added feature to this website is the ability to order this free course for others to take. At that point, you can then monitor their progress and view when they have completed it. It’s mandatory for all team administrators (head & assistant coaches, team managers and team assistants) to take the U.S. Center for SafeSport training and education online course.  The course takes about 90 minutes to complete.  

The correct certificate will read: For the Successful completion of SafeSport Trained.

Upload to your Calsouth file the certificate you receive at the end of the class.  The SafeSport Trained certificate is uploaded by using the same process as uploading the Concussion Education Certificate.  Below is a YouTube tutorial video link.

Do not wait until the last minute to take the class, take it now to avoid the rush (and slow to load problems) that often occur when everyone waits until the last minute to complete.
